The new service form must be completed.

In addition:
          OWNERS--There is a $25.00 service fee. The form may be faxed or e-mailed to the office. The service fee will appear on your first bill.
          RENTERS--If you are renting you will need to bring your rental agreement and an ID card with you to the office.
                                There is a required $75.00 deposit and a $25.00 service fee.
                                You can fax the rental agreement, new service form and ID card to the office.
                                The deposit and service fee will appear on your first bill.
                                The new service form has your SSN, DO NOT e-mail the form to our office.

Discontinue Service
DISCONTINUE SERVICE FORM(PRINTABLE)     ONLINE FORM
A discontinue service form must be completed.

You will also need to bring an ID card with you.
You can fax or e-mail the form with a copy of your ID card to the office.

New Tap
NEW TAP FORM    USERS AGREEMENT     FINANCING AGREEMENT FORM
You must come into the office.
Click the form title above to download the form as a PDF, print it, complete it and bring it with you to the office.
You will be given a flag to mark the location where you want the water meter box to be installed.
A new water tap costs $1,000.00. The new tap fee can be paid at one time or billed monthly for 4 months with a $500.00 down payment.
If you would like to be billed for the new tap fee please complete the Financing Acknowledgement form listed above.
